All youth must receive a copy of the Youth Guidelines/Permission Forms packet, review it with their parent/legal guardian, receive all necessary signatures and submit it during on-site check-in at the Summit.

Forms should NOT be sent directly to DSHS, DBHR or CASAT.   Bring forms to the Summit and submit during on-site check-in. Thank you.
